Home Care & Hospice Services
Founded in 1978, Lutheran Home Care & Hospice is a not-for-profit, faith-based home care and hospice agency dedicated to a mission to serve and care. As a ministry of Lutheran Social Services of South Central Pennsylvania, Lutheran Home Care & Hospice provides home health care, home health telemonitoring, hospice and in-home support services throughout south central and northeastern Pennsylvania and northern Maryland.
In-Home Support Services – For those who need non-medical help to remain at home. Services range from one hour to 24 hours a day. Services include bathing and dressing, meal preparation, housekeeping, laundry, transportation and shopping, labs and companionship.
Home Health Care – For those who need medical care at home. We provide skilled nursing, physical therapy, occupational therapy, speech therapy, phlebotomy, personal care and medical social services.
Home Health Telemonitoring – For those who want peace of mind knowing that someone is monitoring their health status daily. Home health telemonitoring uses innovative technology to collect vital signs and monitor health status from the comfort of your home. It allows detection of health problems before they become serious issues which could require emergency care or hospitalization.
Hospice - Our hospice team includes skilled professionals and volunteers who are dedicated to caring for those living with a terminal illness and their families. We help to manage pain and symptoms so the client can concentrate on important issues and live the last months, weeks and days to the fullest.
